CW Workbench VxWorks Simulator Permanent License Application Form

This document is a form for applying for a permanent software license for Mitsubishi Electric's CW Workbench VxWorks Simulator (CW-Sim). It requires the applicant to provide host machine information (Host Name, Host ID for DISK or MAC), License Authorization Code (LAC), product ID, and company details, and submit the completed form via email to obtain a permanent license file within five business days.
